Hospital Inpatient Payment

CMS’s FY 2027 IPPS/LTCH PPS final rule is the dominant inpatient-payment development, resetting MS-DRG weights, standardized amounts, LTCH policy, quality reporting, eCQM expectations, and graduate medical education provisions in ways that will flow directly into hospital margin assumptions and service-line economics under Medicare FFS and risk-based contracts. For ACOs, MA plans, and health systems in downside-risk arrangements, the rule heightens the need to reprice inpatient utilization forecasts, reassess transfer/post-acute strategies, and align documentation, quality, and clinical redesign priorities—especially as inpatient rate updates and relative-weight shifts can change the ROI of avoidable-admission, site-of-care, and specialty episode strategies.
