Behavioral health VBC activity is centering on access, integration, and utilization oversight: RAND’s design lab for expanding medications for opioid use disorder in community mental health centers points to practical delivery-model changes needed to bring MOUD into safety-net behavioral health settings, with implications for Medicaid managed care, Certified Community Behavioral Health Clinics, and ACO referral networks (RAND MOUD design lab). At the same time, Optum is pushing a “new playbook” for behavioral health payment integrity, signaling payer concern that rapid network expansion, tele-behavioral health growth, and variable coding/documentation could collide with value-based contracting unless quality, access, and fraud-waste-abuse controls are redesigned together (Optum payment integrity). The near-term tension for health systems and ACOs is that philanthropic capacity-building—such as Children’s Mercy’s $150M pediatric mental health investment—may expand access faster than payers can align reimbursement, risk adjustment, and outcomes measurement for behavioral health integration.
