The Trump administration is moving Medicaid drug pricing toward state-enabled “most favored nation” purchasing: President Trump said all 50 states, DC, and Puerto Rico will participate in the GENEROUS program, while CMS has reportedly offered manufacturers exemptions from Medicare pricing demonstrations—Globe and Guard—if they provide additional Medicaid rebates, signaling a shift toward cross-program rebate leverage rather than standalone Medicare-only tests (MFN Medicaid participation; Globe/Guard exemption plan). For VBC stakeholders, the strategic issue is whether these rebate structures create durable affordability for high-cost drugs or simply reallocate savings across Medicaid, Medicare, and manufacturers while leaving provider risk models exposed to specialty-drug volatility.

At the delivery-model level, Indiana’s participation in the federal BALANCE model to cover GLP-1 obesity medications points to a growing willingness to tie Medicaid coverage of high-cost chronic-disease drugs to structured population-health models, while private-sector bundled payment efforts for CAR-T and BiTE oncology therapies show payers and vendors trying to wrap predictable episode pricing around therapies that can otherwise destabilize oncology risk arrangements.
