Kidney Care Models

Value-based kidney care is moving from CMMI-led Medicare experimentation toward broader risk-aligned specialty partnerships, with Blue Cross NC’s Advanced Kidney Care program being positioned as a commercial-market template for CKD/ESRD total-cost and outcomes accountability through nephrology partnerships, care management, and earlier intervention (HCTTF case study). At the same time, Kidney Care Choices/CKCC infrastructure remains strategically important for organizations such as Strive Health, while AI-enabled risk stratification, e-alerts for guideline-directed therapy, and screening-to-intervention partnerships like Renalytix–Carna are reinforcing a shift toward upstream CKD identification and tighter population health management. For ACOs, payers, and health systems, the near-term tension is whether kidney specialty models can integrate cleanly with broader total-cost-of-care contracts without duplicating attribution, care management, or savings accountability across nephrology groups, primary care, MA plans, and ACOs.
