Payer strategy is shifting further toward owned or tightly controlled distribution, benefits, and care assets: Costco’s Medicare Advantage partnership with SCAN Group signals continued retail-channel entry into MA, while Brookings’ mapping of vertical integration shows insurers consolidating influence across PBMs, providers, pharmacies, and data assets—raising network leverage and attribution implications for ACOs and health systems competing for aligned lives (Brookings). At the same time, employers and purchasers are hardening against premium trend, with growing support for hospital price caps and renewed scrutiny of prior authorization and “middlemen,” creating pressure on payers to prove that utilization management and narrow/high-performance networks actually improve outcomes rather than simply shift costs (HFMA). For VBC stakeholders, the strategic tension is clear: payer-led vertical integration and retail MA expansion can accelerate population health management and steerage, but they also intensify contract pressure on independent providers, ACOs, and health systems unless those organizations can demonstrate measurable total-cost-of-care savings, access, and quality performance.
