CMS is exerting unusually tight federal control over the $50B Rural Health Transformation Program, including clawback threats, pushing states toward lower-risk delivery redesign strategies such as downsizing inpatient capacity, shifting services outpatient, and consolidating rural care through regional networks rather than funding broad hospital stabilization. That pressure is accelerating rural hospital alliances and clinically integrated networks as a VBC infrastructure play—giving small providers scale for care management, contracting, data reporting, and shared-risk participation—while creating tension between state-led “transformation” plans and CMS oversight under Administrator Dr. Mehmet Oz.
