Specialty Payment Models

Specialty payment model activity is clustering around kidney care and episode-based care: the Health Care Transformation Task Force is pushing for a longer-term accountable kidney care model, using Strive Health’s CKD/ESKD approach to show how earlier identification, multidisciplinary coordination, and risk-aligned nephrology partnerships can reduce total cost while improving outcomes (kidney care case study). At the same time, CJR-X commentary is reinforcing the operational importance of post-discharge pathways in bundled payments, while CMS’s proposed dialysis payment and quality updates keep renal providers focused on how fee schedule changes and quality scoring will interact with value-based kidney arrangements. For ACOs and specialty groups, the near-term tension is model selection and risk management: LEAD acceptance letters are out, Participant TIN lists are due to CMS in early August, and organizations applying to both LEAD and MSSP must choose before final rules and LEAD financial methodology are fully known (Wakely LEAD analysis).
