Executive Summary
Medicare Advantage is concentrating around high-rated plans even as plan choice shrinks, and CMS is pressing harder on drug prices while states rebuild Medicaid's actuarial and care-coordination foundations.
Star Ratings concentration is now the defining MA dynamic. HFMA reports that more than 70% of MA-PD enrollees sit in contracts rated four stars or higher under CMS's 2027 release, positioning those plans for quality-bonus payments and higher rebates in 2028. Medicare market contraction is the counterweight: KFF finds 2027 MA-PD availability varies widely by county, with a modest decline in the share of enrollees offered more than 40 plans and major insurers exiting far more counties than before, alongside a companion brief cataloguing the premiums, deductibles and cost sharing beneficiaries still shoulder. On Drug Pricing / Negotiation, AAPC reports that CMS has finalized its GLOBE drug payment model, while AJMC reports that PhRMA has sued to block the agency's Most Favored Nation model for Part B, setting up a legal test of CMS's pricing authority.
Medicaid rate setting is under strain. Wakely argues that shifting enrollment, acuity, utilization and provider markets demand more transparent, timely and actuarially defensible capitation processes. On Care Coordination, NASHP examines state approaches to enhanced coordination for children with chronic and complex needs in Medicaid managed care. In Shared Savings / Loss, Medical Economics offers guidance on distributing shared savings without fracturing physician groups, and separately warns that Medicare is underestimating the true cost of joint replacements, a gap with downstream consequences for primary care.
